2. Does anyone in the home/office have any symptoms of Covid-19, such as:
3. In the last 30 days, has anyone in the home/office been asked to isolate or quarantine due to Covid-19?
4. In the last 30 days, has anyone in the home/office been tested for Covid-19?
If yes, what was the result?
5. In the last 14 days, has anyone in the home/office been in close contact with a confirmed or probable case of Covid-19?
6. When the movers arrive, all occupants of the home/office agree to wear a mask and remain 6 feet from the moving crew.
7. There will be other service professionals in the home/office during the move. If yes, I agree to ask the service professional(s) to wear a mask while moving crew is in the building.

8. Invoicing and Payment – A written invoice will be provided upon completion of the job. Payment is due at that time. Presently, we are not accepting credit cards. Although cash is accepted, the crew does not carry change. Please ensure e-transfer limits are sufficient in advance of the move.
